$90.00 For Instructional through Senior (Plus Uniform Cost of $45.00)
After November 5th a $25.00 Late Fee Can Be Charged
Instructional (7-8)
Minor (9-10)
Intermediate (11-12)
Junior (13-14)
Senior (15-17)

There will be a $10.00 returned check charge on any check deposited in the account of CHSA and returned for any reason.
Refund requests will only be considered prior to the first practice. Partial refunds for extenuating circumstances may be considered on an individual basis.

1. If your child has never played a sport for CHSA, please bring a copy of their Birth Certificate.
2. Please bring total fee (see fees above). You can pay via cash or check. Please make checks payable to CHSA.
3. If you are new to CHSA basketball, please complete and print the above forms (3 total) and bring them to signups.

If your daughter played basketball for Clover Hill last year, we will have preprinted forms for you at signups.
There is no need to print the above forms if your daughter played last year.
