1. If your child has never played a sport for CHSA, please bring a copy of their Birth Certificate.
2. Please bring total fee (TBD). You can pay via cash or check. Please make checks payable to CHSA.
3. If you are new to CHSA basketball, please complete and print the below forms (3 total) and bring them to signups.

If your daughter played basketball for Clover Hill last year, we will have preprinted forms for you at signups.
There is no need to print the above forms if your daughter played last year.

9-10 - Minors
11-12 - Intermediate
13-14 - Juniors
15-17 - Seniors
